Approximately 41,000 individuals were affected by the Legacy Treatment Services data breach, and breached data may include, but is not necessarily limited to: First and Last Names, Bank Name, Biometric Data, Card CVV Expiration Date, Clinical Data, Credit / Debit Card Number, Date Of Birth, Doctor’s Name, Driver’s License Number / State Id Number, Email Address, Financial Account Number, Health Insurance Data, Login Data, Medical Diagnosis, Medical Record Number, Medical Treatment/Procedure Data, Passport Number, Patient Account Number, Phone Number, Pin/Security Code, Prescription Data, Routing Number, Social Security Number, Treatment Cost Data, Treatment Location, and Treatment Type.
On August 22, 2025, Legacy Treatment Services and Community Treatment Solutions disclosed a data security event that compromised personal details, including protected health information. The breach stemmed from network connectivity issues, prompting the entities to isolate systems and hire experts for an inquiry. The probe, finalized on July 18, 2025, identified unauthorized entry and data extraction from October 6 to October 11, 2024.
Notifications to impacted parties began on August 22, 2025, with instructions for safeguarding personal data and free credit monitoring for those whose Social Security numbers were involved. The organizations urged steps such as fraud alerts, credit freezes, report reviews, and account surveillance. They pledged improvements to security protocols and privacy safeguards, while setting up a dedicated helpline at 1-877-733-9775 for inquiries, operating weekdays from 9:00 AM to 9:00 PM Eastern Time, holidays excluded.
As a result of the data breach, Legacy Treatment Services is offering 12 months of free credit monitoring and/or identity theft protection services to some affected individuals.
