Decisely set up a toll-free call center to answer questions, available Monday through Friday from 8:00 a.m. to 5:30 p.m. Central Time at (866) 461-3640.

As a result of the data breach, Decisely Insurance Services is offering free credit monitoring and/or identity theft protection services to some affected individuals.
