Data Breach Class Action
August 12, 2022, Attention: victims of the Onyx / iCare data breach.
On or about June 28, 2022, Onyx discovered that its computer systems, and the iCare data stored thereon, were targeted in a cyberattack. It appears that Onyx lost control of its computer systems for a period of time, and subsequently regained control of its computer systems on or about July 7, 2022. A data breach investigation determined that an Onyx / iCare server may have been accessed in the data breach beginning on or about March 29, 2022, and ending on or about June 28, 2022. On or about July 15, 2022, it was learned that some sensitive healthcare related information may have been involved in the data breach.
Approximately 96,814 individuals were affected by the Onyx / iCare data breach.
Breached data may include, but is not necessarily limited to: name, date of birth, address, phone number iCare member ID number, Medicare ID number, dates of service, names of providers.

Onyx Technology, LLC ("Onyx") is a data management and data sharing vendor of Independent Care Health Plan ("iCare") -- iCare uses Onyx's products to allow its patients to access their electronically stored healthcare data.
