Independent Living Systems states that on or about July 5, 2022, it first began experiencing accessibility issues on its computer network. Independent Living Systems claims that upon detecting the accessibility issues, it immediately began investigating the problem with the help of independent third-party computer security experts. According to Independent Living Systems, the cybersecurity experts determined that unsanctioned individuals gained access to Independent Living Systems' computer network from on or about June 30, 2022, to on or about July 5, 2022. It was also learned that sensitive and personal information stored on the Independent Living Systems network was accessed and exfiltrated by the unsanctioned individuals, and additional data was potentially accessed and viewed by these individuals.

Approximately 4,226,508 individuals were affected by the Independent Living Systems data breach.

Breached data may include, but is not necessarily limited to: patient name, Social Security number, date of birth, address, state identification data, driver’s license information, financial account data, Medicare or Medicaid identification information, food delivery data, health insurance records, mental condition and treatment records, physical condition and treatment records, medical record numbers, admission date, discharge date, billing/claims data, diagnosis codes, diagnosis data, and prescription records.
